This is per pair of lenses (540, 580, and Lifestyle Lines).  For a 3 Lens Kit with Rx, you must add x3 Prescription Rx's to your order

Selections

• Progressive  + High Index Lens (Pair) + Anti Reflection Coating 
• High Index is for Prescriptions over 3+ and not in a 580
